mailed back to us before we can ship your order.
Calling your doctor after you place your order with us, and letting him or her
know that we are sending an authorization form, willl allow the doctor
to provide the necessary documentation in a timely manner, thereby
expediting your supply order.

Prescription Authorization Request form
for the NationsHealth Pharmacy. This form must be signed by your doctor prior
to shipping your medications.
